MEMBERSHIP PRICING

Following the initial term of your membership, AcuHaus will give you 30 days advance written notice of any increase in the monthly payment under your membership agreement.

APPOINTMENT CANCELLATION

You will be charged the full price of your acupuncture appointment for late cancellations of less than 1 hour before your appointment. In order to avoid the cancellation fee, please call or text AcuHaus at (952) 222-8158 more than 1 hour before your scheduled appointment. Massage/Bodywork appointments require 5 hour notice of cancellation to avoid the cancellation fee.

BILLING

You agree to the pricing structure that you have selected and agree to make the required payment for the service selected. Single session purchases require payment before services are rendered. Auto-payments are set for the day you buy your contract.

MEMBERSHIP BILLING

You agree to automatic monthly billing for your Acupuncture Membership. Billing is on the same date each month as your date of sign up. Acupuncture Membership in AcuHaus entitles you to 2 (or 4 with a family membership) acupuncture sessions monthly. These acupuncture sessions may be used in the month that they are paid for and 8 acupuncture sessions will accumulate if unused.

INSUFFICIENT FUNDS/ INABILITY TO PAY

AcuHaus will send an email notification after a credit card is declined and automatically resubmit after 3 days.

MEMBERSHIP/SESSION TRANSFERABILITY
Memberships and/or services associated with the Acupuncture Memberships are non-transferrable, non-exchangeable, and are to be used only by the member on the agreement. Acupuncture sessions may not be applied to any other services or products.

MEMBERSHIP CANCELLATION

You may cancel your membership after the initial month by completing our cancellation form in person or on-line. AcuHaus requires a 30-day notification to process your cancellation; there may be another payment due in this 30-day notification window. Upon cancellation any unused acupuncture sessions or other specials included with your membership will expire 90 days from the cancellation date.

If you are dissatisfied with any purchase or service at AcuHaus please bring it to our attention.

THESE TERMS AND CONDITIONS MAY BE MODIFIED AND APPLY AT EACH VISIT TO ACUHAUS.

SERVICE DETAILS AND REQUIREMENTS

To the best of AcuHaus’s knowledge, only professional Licensed Acupuncturists who comply with state, city, and/or local licensing or certification requirements are hired by AcuHaus. Licenses are displayed for your review in the clinic. You understand that the services provided are not a replacement for medical care, should not be construed as a substitute for medical examination, diagnosis, or treatment, that no medical diagnosis will be made, and that you should see a medical provider for any medical issues you may have. It is your responsibility to inform AcuHaus of any pre-existing conditions, limitations, or specific sensitivities.

Male/female genitalia will not be exposed or treated at any time. Modest draping will be used during each of your services. If you do experience discomfort or pain or are uncomfortable for any reason during a service, you agree to immediately ask the Licensed Acupuncturist to adjust the manipulation, draping, pressure, heat, or environment (or, if you prefer, you can ask the Licensed Acupuncturist to end the service at any time). If the Licensed Acupuncturist is unable to relieve your discomfort after you request an adjustment, you will inform the Licensed Acupuncturist you would like to end the service immediately. If you have any concerns about your Licensed Acupuncturist, you agree to bring it to the attention of the AcuHaus ownership immediately following your service. Inappropriate or illegal conduct will not be tolerated in any manner. AcuHaus may, in its sole discretion, refuse or discontinue service if it determines such service may be unsafe or cause discomfort for you or if you engage in any inappropriate conduct as determined by AcuHaus in its sole discretion.

OTHER TERMS AND CONDITIONS

AcuHaus reserves the right to terminate or refuse to renew your agreement for any reason not prohibited by law including, but not limited to, an unsatisfactory payment history. AcuHaus reserves the right to collect at any time any delinquent or outstanding balance(s) that have not been paid for any services provided or monthly payments owed. For purposes of identification and billing, you agree to provide AcuHaus with current, accurate, complete, and updated information including your name, address, telephone number, and applicable payment data. You agree to notify AcuHaus promptly of any changes in your information, including your payment data. AcuHaus may delay enforcing any of its rights without losing them. AcuHaus can enforce the membership agreement against your heirs and legal representatives and may assign or transfer such agreement or any rights under it without notice to you, except as otherwise required by law. Your rights or obligations under the membership cannot be assigned by you to anyone else without the AcuHaus’s prior written consent. In the event of the AcuHaus’s closure of their location, you will be refunded for all outstanding services. The invalidity or unenforceability of any provision of your membership shall not affect the validity or enforceability of any other provision of the agreement, which shall remain in full force and effect. You understand and agree that your membership does not grant you the privilege of exclusive or preferred access to the AcuHaus’s location. You or any other individual may obtain access to and purchase any and all services

offered by AcuHaus without entering into an agreement, and you understand that your membership only entitles you to the benefits set forth therein.

By voluntarily signing the membership agreement, I confirm that I have read, or have had read to me, and understand the above membership contract agreement: including the pricing structure, appointment cancellation, billing policy, transferability, membership cancellation, and terms and conditions listed.
